The distinction matters because it decides what can go wrong, and people arrive expecting the fault list of a refrigerator ice maker.
A refrigerator ice maker is a small mould inside a freezer that exists for another purpose. It fills for a fixed number of seconds, freezes a batch slowly using cold the freezer is producing anyway, and drops it into a bin. It has no compressor of its own, no water pump and no drain.

It has a compressor, a condenser and a fan; a water pump that circulates water over a chilled plate or into a mould; a harvest mechanism; and a drain to carry away what melts. The Drain, Which Causes Most of It
This is the section worth reading, because it explains almost everything people describe as a dirty or failing built-in ice machine.
An ice machine makes ice and then keeps it in an insulated but unrefrigerated bin, so some of it melts continuously. That meltwater has to leave, and it leaves through a drain: either by gravity to a suitable point, or through a small pump where gravity is not available.
When that drain does not work properly, water stands in the bottom of the bin. It sits there at a temperature well above freezing, in the dark, with the machine adding fresh ice on top. What grows in it is the pink or grey film people find and assume means the machine is unclean inside. It is, and the reason is the drain rather than the ice.
Three things go wrong and they are worth telling apart.
A gravity drain with no fall. The commonest installation fault on these machines. An undercounter unit in an island or a bar is often a long way from a drain point, and the run ends up nearly level. A nearly level drain does not drain. That is not something a service visit fixes for good: the run itself has to be corrected, or a pump fitted.
A blocked drain. Slime and mineral deposit narrow the line over the years, particularly if cleaning cycles have not been run. Clearing it is straightforward and it will recur unless the cleaning routine changes.
A failed drain pump. Where one is fitted, it can stop silently. The machine keeps making ice until the reservoir fills, then stops, and the complaint arrives as “it stopped making ice” with no mention of water at all. The Cleaning Cycle Nobody Runs

Two separate processes are involved and both matter.
Descaling deals with mineral deposit on the evaporator plate or in the mould, in the water pump and in the distribution tube. Scale insulates the freezing surface, so a scaled machine takes longer per batch and produces softer, cloudier ice before it produces less of it. This is the half that affects performance.
Sanitising deals with the biological film in the reservoir, the bin and the drain path. This is the half that affects taste and appearance, and it is the one connected to the drain problem above.

Cloudiness in ice is trapped air and dissolved minerals. When water freezes quickly from all sides at once, as it does in a mould, everything dissolved in it is driven inward and trapped as the last of the water freezes. That is the white core in ordinary ice, and it is physics rather than a water quality problem.
Clear ice is made by flowing water continuously over a chilled plate so it freezes in thin layers from one side, carrying dissolved material away instead of trapping it. What is left is dense, slow-melting and transparent.
The consequence is that a clear-ice machine depends on a pump and an even flow across that plate, so a clear-ice machine producing cloudy ice is a real fault rather than a cosmetic complaint. It usually means the pump has weakened or the distribution over the plate is uneven, so water is standing rather than flowing. That is the earliest sign these machines give, and it is worth acting on.
If cubes are getting smaller and cloudier at the same time, look at the water side before the refrigeration: a partly closed supply valve, an overdue filter or a weakening pump all reduce flow across the plate, shorten the freeze and trap more air.
Outdoors, and Why Rating Matters

Either way, ventilation is not optional. Ice is not inert. It sublimates slowly, meaning it evaporates directly from solid without melting, so cubes left sitting shrink, lose their edges and fuse to their neighbours. And it absorbs odours from whatever air surrounds it, because a large mass of ice has an enormous surface area for its volume.
In a bin filled for a party in February and left until April, both of those have had two months to work. What you find is a solid, slightly shrunken mass that tastes faintly of the machine, and it is easy to conclude the appliance has gone wrong.
There is a second mechanism on the refrigerator side. Warm air holds far more water vapour than cold air, so every freezer door opening admits moisture that freezes on the exposed cubes in the bin, adding a film of frost each time. After a week or two those films bridge the cubes into one block, the dispenser jams or grinds, and the ice maker gets the blame.
Neither is a fault. Where a bin has already fused, empty it and let it warm to room temperature so all the bridging frost melts rather than just the surface, then dry it and put it back. Chipping at the block leaves the frost that caused it and it re-fuses within days.
