Nobody Plumbed a Refrigerator in 1958

The valve that is not fully open. Somebody closed it during other work and reopened it partway. An ice maker fills for a fixed number of seconds rather than to a level, so a partly open valve does not stop the ice, it makes the cubes thin. That is why this one goes unnoticed for months.

The kink behind the appliance. Push a refrigerator back hard against the wall in a tight midcentury alcove and the line gets crushed against it. Same effect as a partly closed valve, plus the tubing work-hardens at that point until it eventually splits.

Chipping at the block leaves the frost that caused it.

Where the Water Actually Ends Up When a Retrofit Leaks

This is worth a short section because a leak from a retrofitted ice maker line rarely appears where the leak is, and people spend a long time looking in the wrong place.

The line runs behind cabinetry, along a toe kick or under a floor. When it weeps at a fitting, the water follows whatever surface it lands on until something stops it. When It Is Not Worth Repairing
